Above these images, you will see more headings, such as; "New Patient Registration",  "Health History", "Traveler's History" and "Fever Chart".  If you are new to our practice or have not been seen in 2 years, you will need to fill both the New Patient Registration and Health History forms out upon presentation to the office.  You may click on either of these headings to find printable forms.  We encourage you to print these forms before your visit and fill them out.  This can really help at the time of your first visit.  With all this information already completed, your wait and our schedule will be much better!

The Fever Chart is available for you to use to document your temperatures before you see us or as a part of our ongoing management.  You may print this form out as well and please note the directions listed on method and timing of temperature measurement. 

PLEASE NOTE:  We are unable to prescribe for any patient unless we see that  individual professionally.   Although it would be easier and faster without a visit to the office; it is not considered standard practice, nor is it considered legal.  We do try to minimize your trips to see us.  If you are an established patient, we may be able to call in a prescription in some (very few) cases.  As a specialty office, there are very few cases that are not under current evaluation.  Most of our patients need to be examined or educated limiting our ability to manage them by phone safely and competently.  We do not wish to inconvenience you, however it is our committment to try to take care of our patients in the right way.  This may mean you will need to be seen to be cared for correctly.
